Taking Action

Before we can welcome your loved one into our home, there is some paperwork that will need to be completed through the State of Minnesota:

Checklist

To begin the process of using our services immediately:

Contact your County Department of Human Services and request a Case Manager in the Developmental Disabilities Unit.
The County will assign a case manager to start the process for finding Care.
The Case Manager will assist in getting you a financial case worker to get proper approvals.
Let your case manager know that you are interested in Northeast Residence, Inc. and he/she will organize a tour.
